SS-31

also known as elamipretide or Bendavia, is a synthetic tetrapeptide that selectively targets and protects mitochondria to enhance energy production and reduce oxidative stress. In September 2025, the FDA granted accelerated approval to elamipretide (branded as Forzinity) as the first treatment for Barth syndrome. 

 

FDA Approval and Medical Status

  • Approved Indication: Specifically approved for adult and pediatric patients (weighing ≥ 30 kg) with Barth syndrome, a rare genetic mitochondrial disorder that causes heart failure and muscle weakness.
  • Regulatory Mechanism: The approval was "accelerated," based on improvements in muscle strength; a post-approval trial is required to confirm long-term clinical benefits.
  • Availability: It is a prescription medication when used as Forzinity. While widely sold online as a "research chemical," these versions are unregulated and not intended for human use. 

 

How It Works

SS-31 differs from traditional antioxidants by directly stabilizing the internal structure of the mitochondria. 

  • Cardiolipin Binding: It binds with high affinity to cardiolipin, a lipid essential for the structure of the inner mitochondrial membrane.
  • Energy Production: By stabilizing cardiolipin, it optimizes the electron transport chain, which enhances ATP (cellular energy) production.
  • ROS Scavenging: It reduces the leakage of electrons that leads to the formation of harmful reactive oxygen species (ROS), thereby lowering oxidative stress. 

 

Potential Benefits and Research Areas

Beyond Barth syndrome, SS-31 is extensively studied for conditions characterized by mitochondrial decay: 

  • Age-Related Decline: Research suggests it may reverse skeletal muscle deficits and improve exercise tolerance in aging.
  • Organ Protection: Preclinical models show promise in treating acute kidney injury (AKI), heart failure, and diabetic nephropathy.
  • Neuroprotection: It is being investigated for its ability to rescue cognitive deficits and protect neurons in conditions like Alzheimer's and traumatic brain injury. 

 

Safety and Side Effects

Clinical trials indicate that SS-31 is generally well-tolerated: 

  • Common Side Effects: The most frequent issues are mild injection site reactions, such as redness, itching, or pain. Other reported effects include headaches and dizziness.
